1. IA's benefit from having larger claims and making sure that they catch everything the first time.  If you can close the case on site and educate and help them justify your line items, everyone wins. Case isn't dragged out, insurance company saves on admin costs, contractor saves time before getting paid and IA can move on to additional cases.
2. Some line items are automatically denied because that is within their guidelines of which they have no control over.  If you want to have that line item paid, state a case they can take to their managers or take your case directly to the carrier if that if they're the only ones with the power to negotiate.
3. Low pricing averages are more likely due to TPA work and contractors accepting minimums
4. How everything goes depends on the level of documentation, justification and true need.
